Structure and Operations:

• Employees complete a confidential health risk questionnaire along with several health measurements to receive year-end bonuses when they maintain or improve their weight.   
• The employer strives to provide a healthy work environment by:
  • Providing reimbursement for fitness-related items, including fitness center dues
  • Allowing flexible schedules to provide time for exercise and other health promoting activities
  • Establishing on-site one-on-one health coaching and having a corporate chaplain
  • Encouraging the consumption of fruits and vegetables
• Employee bonuses are tied to maintenance of a healthy weight and additional incentives are provided to encourage participation in wellness activities
